Safeguarding that learns from tragedy

When safeguarding systems fail, there must be accountability, transparency and a commitment to learning from mistakes. Every child deserves to grow up safe, loved and protected, and no child should die before anyone asks whether more could have been done.

What we're asking for

  • Act on warning signs raised by family members, professionals or the public
  • Take concerns from non-resident parents and wider family seriously
  • Publish clear learning from serious case reviews
  • Make opportunities to intervene a matter of record, not hindsight
